Sure Accountable Care Organizations ACOs Can Save Money But Can They MAKE Money

ACOs at work.
According to this Bloomberg news release, some of Medicares Accountable Care Organizations (ACOs) are already achieving cost savings. Mt. Sinai and Coastal Carolina are reducing emergency room visits while Hackensack is reducing costs.

All three institutions are using two key ingredients:

1) information technology-based risk stratification to identify the persons at greatest risk and

 2) dedicated full-time nurses who perform telephonic and in-person outreach, coordinate care and provide patient coaching that, in turn, is tailored to that risk.

To the DMCB, the good news is that ACOs are using the two approaches that define modern-day disease and population health management. That industrys success will be Mt Sinais, Coastal Carolinas and Hackensacks success.

The bad news is that the news release only addresses half the question: did any savings exceed the institutions cost of the risk stratification and the nurse-FTEs? If the early answer is no, then avoided ER visits and reduced costs could turn out to be much like Governor Christies lap band: so far so good but its still risky and could ultimately be all for naught.

And on an unrelated note, this just-published New England Journal article makes note of "not made in America" health care innovations from overseas that could hold important lessons for the United States. In particular, the authors point out that Germanys DRG hospital payment system includes 30-days of post-discharge care and includes the physician payment. Readmissions within that 30 day window are, with a few exceptions, not covered and physician payment is possible because docs are often employees of the hospitals.

"Interesting!" says the DMCB, but is reminded that Germany is hardly a model for reducing inflationary cost trends.   It also specifically recalls hearing Germanys Minister of Health, Daniel Bahr, express impatience with his countrys DRG system just last week. He criticized it for not advancing enough quality in his keynote address at the HauptKongress in Berlin.

"Historically, most of the injuries caused by the waves rarely reported. This is the first study to record all types of injuries that occur due to waves in the ocean," said Paul Cowan of Beebe Medical Center in Lewes, as reported by U.S. News.

I have been inspired by the comments of a recent reader to get myself centered on my program again. Tricia and I have been sharing back and forth on the blog post just previous to this one and I must say that it really is nice to feel the power of partnership with her. It just seems so much richer to know that she is there, in the same boat that I am in, and we are not alone in our struggles. I desire for both of us to be successful. We share life-long weight problems, pre-diabetes, and the desire to make the kind of lifestyle changes that will help us rid ourselves of the diagnosis of "pre-diabetes." If you need to get back in the swing, please join us in stepping out on a ten-day getting started commitment.

My last official post was dated the day before Thanksgiving in November and I had plans to stay on track for Christmas and New Years, too... but somewhere things got foggy for me. Even my daily record has a few "missing days." But the past, is the past. I now must face forward and decide what my next step is.

The advice that I shared is the advice that I need. Here are the summary guidelines that I had come up with to get re-motivated and make a simple ten-day step in the right direction:

1. Read your book again for motivation.
2. Set a date to begin your change.
3. Prepare by removing what you dont need from your house
4. Prepare by providing what you do need for healthy eating
5. Do your plan for ten days
6. Then see where you are and decide what you will do for the next ten days.

1. Read your book again for motivation.

Many of us have been inspired by some book or program or idea that has helped us do the right stuff in the past, but we are no longer in that space right now. One way of getting back into that space of inspiration is to read the same book again. I did that a while back with the "Carbohydrate Addicts Lifespan Program." I was also very inspired by the "30 Day Diabetes Cure" with which I started this blog. I also have been inspired by and gleaned a lot of good information from the vegan culture that is very active on the internet (Ive shared about all this before.) 

If you know of a program that you were successful on, then go back, read it again, and begin to get your mind focused on the right stuff for you. Rekindle your passion. Get reacquainted with the guidelines you already know work for you. Allow yourself to be inspired.

2. Set a date to begin your change.

At some point, often while you are getting re-acquainted with the details, you will feel inspired to begin doing your program again. At that moment, set your date to begin doing the program. It is all well and good to read it... but you will only get results by doing it. Pick a date that is not too far away, but is far enough away that you can make the kinds of preparations that you need to make.

I know that when Ive been off my program for any length of time (be it weeks or be it years) it can seem overwhelming to even consider going back and doing it all again. It is "so much easier" to simply lolly-gag and continue stuffing all the standard stuff into my mouth while pretending that it does not make a difference what I eat. The point is, we begin to lie to ourselves. It is hard to admit that, but the addiction takes over and lulls me into "never-never land" -- you know -- the place where things never change?

Nothing can knock the block off the addiction lies like setting a date to begin. A starting date is concrete. It is a day on a calendar and no matter what your addiction says, that day will show up in your life. It is marching down the daily track in your direction. Your own personal day of reckoning will definitely arrive. If you can see it coming, that, all by itself, will motivate you to begin taking preparatory actions.

3. Prepare by removing what you dont need from your house

The most obvious first step is to get rid of all the stuff that you know you should not be eating, before your start date gets here. You will only make yourself sick if your method of getting rid of it is to eat it -- but this is your life and your choice. The better method of getting rid of stuff is to recognize it for what it is (toxic) and throw it into the trash. Do not leave even one spoonful of ice cream in your freezer. Simply wash it down the sink and toss the box out. You will be making space for the next step in the plan.

4. Prepare by providing what you do need for healthy eating

Using your program as a guideline, purchase the kinds of foods that you know will give your body health. Stock up on good healthy food. Some things will need to be divided into individual healthy meal portions and stored either in the freezer or the rigerator. These will be your new "convenience" foods.

In my case, that means that I need to purchase fresh raw vegetables and salad fixings. I also need to get good protein in the form of eggs, beef, pork, and chicken. Since I also have gout I need to be caul with animal protein (especially fish) but not extremely so. Ive been reading the book "Beating Gout" by Victor Konshin and have discovered that limiting animal proteins will only drop uric acid levels by two points at best, the rest must be done with medication. Ive been on the medication (Allopurinol) for about six months now and have not had a "flare" in quite some time. To me, this means that the guidelines Victor has given are working for me. Im going on Medicare on the first of March and when I get my new doctor (my old one does not take Medicare patients) and my new insurance, I will be having my uric acid levels checked to make sure Im on track.

I am going to be eating my food in the form that God gave it to me with my focus on raw vegetables and cooked meat. Im also toying with the idea of trying the "seventh-day cheat meal" as a standard procedure. I may use my next post, to describe that program in more detail. I have it written out for myself and may simply post it so others can see or use it also. It may be that I am not able to do that. I will find out, once and for all, if I can really do it. More on that another day.

5. Do your plan for ten days

I chose ten days because that is the time increment that was used in the "30 Day Diabetes Cure" and it really worked great for me. What I saw happening was that a ten-day span appeared to me to be a short do-able not-overwhelming time span. It was easy to think of doing something for only 10 days. The first challenge was to stop eating sugar of any kind, no sweets, and no sugar substitutes for 10 days. Then, once I accomplished that task, and saw how easy it really was, and how much better I felt, I simply continued.

Three days would not have been long enough to see the good results you would want. Three days is almost exactly the amount of time it takes to go through sugar withdrawals, so they are the hardest days to accomplish -- but since you have made a commitment, the first three days are done by having made the decision to do it. By day four, things begin to lighten up and you no longer even crave sugary items. From day four on, things get easier -- and they happen almost on their own. No will power is needed after day 4 because your body will no longer be pushing you. So... if you had only chosen to do the plan for three days, you would have shot yourself in the foot and not allowed the change to actually take affect in your body.  You would never have gotten to "easy street." After youve lived the results for ten days, it would be insanity to go back -- but the choice is still yours.

6. Then see where you are and decide what you will do for the next ten days.

By the time you finish the first 10 days the second 10 days will look really easy to accomplish. The hard part is over and the only thing you need to do is keep doing what you are already doing. But you have the option.  That option lets you be in charge of your own life. You decide what you will be doing next. And there it is: commitment -- and your "self" is fully engaged in the battle once again.

If you look at what has happened in your own life and you see a need to tweak something, now is your opportunity -- but do not let that interfere with your next step. Make any adjustments and then make your new commitment to continue on to bigger and better results.

Dont forget the "see where you are" part. On day 11 take stock and ask yourself some questions. Did your blood sugar numbers get better? (This also means that for the 10 days you did this, you kept track of your numbers.)  Did you lose any weight? How do you feel? Where are your pain levels? I have found in the past that my pain levels seem to greatly diminish on day 6. I dont know if other people experience the same thing, but if you do, you need to make a note of your pain levels before and after that initial ten-day accomplishment.

When you answer the above questions, and any others that are pertinent to you and your situation, you will be looking at concrete evidence of your own success. Nothing motivates like success!! Use your success to continue moving you along in the right direction.

Veggie Might Make Your Own Ketchup

Penned by the effervescent Leigh, Veggie Might is a weekly Thursday column about the wide world of Vegetarianism.

For most people, ketchup and French fries go together like peanut butter and jelly. For me, ketchup and jelly can both take a hike. French fries are better with malt vinegar or mayo, and peanut butter is just fine all by itself.

But I also hate bananas and strawberry ice cream, so what do I know?

It’s not that I hate ketchup, lest you think me the Andy Rooney of condiments, it’s just I find it bland and not really worth the effort. I’d rather have a little zing or richness with my fried potatoes.

I only buy ketchup to make BBQ sauce or my Dad’s BBQ slaw. Last week, I had flames on the sides of my face buying a bottle of bland ConAgra tomato sauce, chockfull of high fructose corn syrup and “natural flavors” just to make the BBQ sauce for the seitan bites.

Then it occurred to me that if I can make my own mustard, I could surely make my own ketchup. So I did.

I found several recipes on the InterWebs that looked promising: Homemade Ketchup from Epicurious, Homemade Ketchup from Kiss My Spatula—which was an adaptation of this recipe from Saveur, and this no-cook ketchup recipe from Hillbilly Housewife.

Between them, I came up with my own, and wow!

This ketchup has amazing flavor from the spices, tanginess from the cider vinegar, and just the right amount of sweetness from the carrot and molasses. You can substitute brown sugar—all the recipes called for brown sugar, but I swapped it out.

Homemade ketchup is not necessarily cheaper than store-bought, but it is certainly tastier, and in my opinion, worth the effort. I will never buy ketchup again. And, who knows, I may start eating it on French fries.

Emperor-worthy Tomato Ketchup
Yields 3 cups/48 1-tbsp servings

28 oz can crushed tomatoes
1 tbsp tomato paste
1/2 cup cider vinegar
1 tbsp olive oil
1 medium onion, finely chopped
3–4 cloves of garlic, minced
1 rib of celery, finely chopped
1/2 large carrot, finely chopped
1/3 cup molasses
1 tsp sea salt
4 cloves
1 bay leaf
1 dried red chili
4 allspice berries
1 cinnamon stick
1/4 tsp brown mustard seed

1) In a medium saucepan on medium heat, sauté chopped onion for 3 minutes. Add minced garlic, celery, and carrot. Continue to sauté until soft for another 3–5 minutes.

2) Add crushed tomatoes and stir. Then add tomato paste, vinegar, molasses, and salt, stirring well.

3) Make a spice sachet with a piece of cheesecloth. Place cinnamon, allspice berries, dried chili, bay leaf, cloves, and mustard seeds on the cheesecloth and tie tightly. Drop into pot of tomatoes.

4) Simmer for 1 hour or until desired consistency.

5) Remove from heat. Allow to cool for 10 – 15 minutes. Remove spice sachet and pour mixture into blender. Puree until smooth.

6) Chill for 2 hours to allow flavors to blend.

7) Serve as desired or eat with a spoon. Your choice.

Veggie Might Make Your Own Almond Milk

Complete the following word problem and show your work:
You purchased a 4-lb bag of almonds for $10.99 at the Indian market near your friend T’s house. Three weeks later, you still have 1/2 of the bag left. You are worried you may not eat all the remaining almonds before they go rancid because you forgot to stick the bag in the fridge sooner. What can you do with 1 cup of the remaining almonds?

Sample response:
Make almond milk.

1) Cover 1 cup of almonds with water and soak overnight.

2) Drain, rinse, and place almonds in a blender with 3 fresh cups of water. Puree until the almonds are chopped finely.

3) Using cheesecloth or a fine strainer, strain the liquid from the almond mash. Press the mash to extract as much liquid as possible.

4) Set aside the mash for later.

5) Strain a second time. If you used cheese cloth, switch to a fine strainer.

6) Save the superfine mash—this is almond paste!

7) Refrigerate the liquid for 24 hours. There will be some separations, so give the almond milk a good shake. Add a more water if it’s too thick. Add a pinch of salt and sweetener to taste.

Making almond milk is very satisfying, if a touch time consuming. But believe me, the almond milk you will make is far tastier and richer than commercial almond milks I’ve tried (though doesn’t keep as long—only about a week in the fridge). No added thickeners!

Plus, you’ll cut down on those aseptic cartons that are difficult to recycle. You can also dry (in a 200 degree oven) the leftover almond meal and use it in baking. No waste!

Commercial almond milk: 3 cups = $2.24

Adult children who are older and have parents addicted to alcohol or drugs have a higher risk of depression than children whose parents do not have the habit of drinking alcohol or illegal drugs.

It was discovered after researchers observed 6,300 adults in Canada. They found that about 312 people into a deep depression last year. Approximately 877 participants who began the study admitted under 18 years old and still living with my parents. They also explained that their parents have the habit of drinking alcohol and using drugs. Drinking alcohol and illicit drugs are recognized to cause problems in the family.

After calculating other factors such as age, gender, and race, researchers at the University of Toronto found that adults who had childhood experiences with parents who have an addiction to alcohol and drugs likely affected by depression twice as high.

"Even after accounting for other factors such as parenting mistakes, parental occupation, and parental lifestyle that is unhealthy, researchers found that parents addicted to alcohol and drugs associated with a 69 percent chance of causing a child depression," said lead researcher Esme Fuller-Thomson, as reported by U.S. News.

Fuller-Thomson explains that the possibility that parental behavior affects the way children deal with depression and stress. Researchers suggested that children who live with parents with alcohol or drug addiction have more contact with other family members such as fathers, grandfathers, uncles, or other.

Too old to drink milk make baby less iron? - Although breast milk is considered as the best nutritious food for babies, but a study shows that babies who are too old to drink milk possible iron deficiency.

"We found that after one year, the longer the baby feeds her mother, the higher the risk for iron deficiency," said researcher Dr. Jonathan Maguire, a pediatrician from the Li Ka Shing Knowledge Institute at St. Michaels Hospital, University of Toronto, Canada.

The study, published online in the journal Pediatrics on April 15, the findings suggest an association between duration of breastfeeding children with their propensity to consume iron deficiency can cause anemia. Iron is an essential nutrient, especially for children. Iron is needed in the development of the nervous system and brain.

World Health Organization recommends exclusive breastfeeding for the child for six months, then introduce them to food ation supporters. Based on research, WHO allow breastfeeding to children aged two years or more.

Researchers looked at 1,650 children aged between one and six years with an average age of three years. No one is suffering from a rare disease. Yet they found a decrease in the level of iron as much as five percent of each additional month of lactation. Researchers also found the same in children who consume too much dairy cows.

"Cows milk does not contain a lot of iron, for that breast milk is better for the child during the first year," Maguire said, as quoted by Health Day News (19/04).

Meanwhile, one of the doctors laRabida Childrens Hospital in Chicago still recommends breast milk for the baby. According to him, the iron in the mother can be given to children more effectively to children growing up. After that, the need for iron increases and the child would have to be supported by other foods.

Recent studies reveal that drinking soda excessively can make rapid tooth decay and cracked. In fact, the adverse effects of soda called as bad as taking narcotics.
