Heartburn is a painful, burning feeling in your chest or throat that occurs due to the regurgitation of stomach acid into your esophagus. Almost one in 10 Americans experience symptoms of heartburn at least once a week? If left untreated acid lux and heartburn can lead to a more serious condition, like gastroesophageal lux disease, or GERD.


It occurs due to improper food consumption and lifestyle habits. However, the good news is that with a little change in your daily habits and some simple natural home remedies; you can easily combat this condition and avoid the uncomfortable burning sensation.

Here is a list of top 30 home remedies to treat acidity and heartburn

01.Take a glass of warm water, add a pinch of hing (asafoetida) and have it every morning to prevent acidity.

02.Eat a serving of cucumber or watermelon 4-5 times in a day to fight against acidity.

03.Fennel helps to relieve abdominal pain, colon disorders, gas, and gastrointestinal tract spasms. It is an effective remedy for acidic stomach conditions (heartburn).

04.Ginger is very useful for digestive disorders. It help to relieve indigestion from rich foods.
Make a fine powder by adding equal amounts of ginger powder, black pepper, dried mint leaves, asafetida (hing), anise seeds, coriander seeds, cumin, fennel and common salt. Dosage: Take about 1 teaspoon of this powder with water, twice a day after meals.

05.It is advised to eat 8-10 glass to water daily. Drinking water gives immediate relief to symptoms like wind, distended stomach and acidity.

06.Chewing bubble-gum also disappears the acidity. This is an easy and best way to get rid of Acidity.

07.For heartburn drink a teaspoon of vinegar. It is because there is not sufficient acid in our stomach to digest food properly, thus gasses come back up and burn our stomach.

15. Take half a liter water add a tsp of caraway seeds and boil it. after five minutes pour the water in a cup remove seeds and drink warm not too much hot. Have the concoction 2-3 times a day for 5-6 days.

16. Take some cloves and cardamom and blend dry to make powder, use the powder as a mouth freshener after every meal. No more acidity and no more bad breath.

17. Drinking milk or eating milk products are helpful in relieving symptoms of acidity.

18. Clove also helps to relieve the symptoms of acidity. Suck one piece of clove slowly to relieve the symptoms. It also helps to reduce the onset of disease that arises due to acidity.

19.A cup of vanilla ice cream or a glass of cold milk also helps to get relieve heartburn and acidity within minutes.

20.Almonds are also a good source for relieving the heartburn and acidity. Eat several almonds when heartburn symptoms persist.

21.Lemons also help to prevent heartburn. Cut a lemon into thin strips and dip in salt. Eat before meals to prevent heartburn.

22.Take some mint leaves and chop these and then boil in a cup of water and sip slowly after meals. If a person drinks Fresh mint juice every day he will never experience acidity.

23.Eating bananas is also helpful in preventing the symptoms of acidity and heartburn. To prevent acidity it is advised to eat a banana daily.

24.A mixture of 2 tsp of natural apple cider vinegar and 2 tsp raw honey in a glass of water before meals fights with acidity.

25.Basil leaves helps to get relief from burning, nausea and gas. Chew some leaves of tulsi to decrease the acidity.

26.Coconut water 4-5 times a day decreases the symptoms of the acidity and the heartburn. Harad (Terminalia chebula) if taken after every meal prevents the symptoms of the acidity.

27.1 tsp juice of chebulic myroblan mixed with 1 tsp of gooseberry juice helps to prevent acidity and heartburn.

28.Another very effective home remedy for acidity is - Boil one cup of water. To this add 1 tsp of Aniseed (Saunf). Cover and leave overnight. Strain the water in the morning, add 1 tsp of honey. When this is taken 3 times a day it prevents acidity.

29.To 1 ½ liters of water add 1 tsp Caraway seeds (shah jeera). Bring to a boil and simmer for 15 minutes. Sip while warm. Take this liquid 2-3 times a day for 5-6 days. This is also very effective home remedy for acidity.

30.Take a glass of water, add a pinch of baking soda and drink it. It gives immediate relief from acidity.

As i told earlier, i baked 3 cookies out of 4 cookies suggested by Vijayalakshmi for our this months Home Bakers Challenge, one among the cookies is this peanut butter cookies. Am an ardent fan of peanut butter and my pantry will always a huge bottle of this nutty butter. This peanut butter is quite versatile and tried many dishes out of it, even i tried adding in our Indian eggplant curries, trust me they tastes fantabulous. I baked this peanut butter cookies immediately once Viji announced her choice of cookies and everyone at home just loved munching this super nutty cookies.

Peanut butter cookies with crunchy peanut butter tastes fantastic and obviously they makes a pect pair with a cup of warm milk. Eventhough am not a breakfast person, i dont hesitate a second to munch these baked goodies for my breakfast, yep i just love it that much. Now you may know how much i love these super nutty butter.To add more nuttiness to this delicious scrumptious cookies, you can also some chopped roasted peanuts too.Sending this biscuits to Gurus Giveaway - Kids Special.

1+1/2cups All purpose flour
1tsp Baking powder
1+1/2cup Crunchy peanut butter
1cup Packed light brown sugar
1/2cup Unsalted soften butter
Milk (as per need)

Preheat the oven to 350F.

In a mixing bowl, beat together the peanut butter, sugar and butter until they turns smooth.

Sift the flour and baking powder together and keep aside.

Gradually add the flour mixture to the wet ingredients and combine,sprinkle milk to turn everything as soft dough.

Pinch off the dough as medium size balls, and roll.

Place it over a baking sheet lined over a baking tray, press the balls with a fork.

Bake the cookies for 15-18minutes until they turns lightly golden.

Cool the cookies on a wire rack and enjoy.

1) Honey and cinnamon.
Honey is a natural antibiotic and works to pull out dirt from your open pores. Mix a tablespoon of honey with ½ a teaspoon of cinnamon and use your fingers to dab it onto your dry skin. Rub it in a circular motion for three minutes and then wash off with warm water.
2) Turmeric and Mint 
Turmeric and mint help to clean out dirty pores. Create a brew of mint tea and allow it to cool. Then, add two tablespoons of the liquid with a tablespoon of turmeric powder and apply it to your face. Leave it on for ten minutes and then rinse off with warm water.
3)Use Eepsom salt
Epsom salt mixed with iodine form a bacteria-fighting combo perfect for removing blackheads. Mix a tablespoon of epsom salt with hot water and a few drops of iodine. Let the salt to completely dissolve in hot water. Then, use a bit of cotton to dab the solution onto your skin, allowing it to dry onto your face. Then gently rinse the mixture off with clean warm water.
4) Egg whites
Egg whites help to tighten pores and pull out any clogged dirt giving you smooth, clear skin. Remove the yolks from two eggs and smear it into a thin layer over your face. Give it two minutes to dry, and then add the remaining whites in a second layer. Give the complete mask an additional 10-15 minutes to dry, or until the whites are smooth to the touch and your skin feels tight. Use warm water to rinse the whites off your face.

5) Fenugreek paste
Make a smooth paste by crushing fenugreek leaves. Spread this paste over your face and allow it to sit for ten minutes, and then rinse off. Besides its numerous other health benefits, fenugreek has shown promising results for removing blackheads. 

6) Oatmeal 
Grind oatmeal into a powder in a blender and then add some rose water. Use this on affected areas for 15 minutes and then wash it off with cold water.

Home made Exfoliator recipes

1) Baking soda
Use baking soda and a bit of water to create a paste, and then gently scrub in a circular motion over the affected surface of your face, dab a thick coating of the paste to the affected area and allow it to dry for 5-10 minutes before rinsing off. Baking soda is one of those magical substances in life that is great for myriad purposes. Besides being a natural cleanser, the fine grain of the powder clears away dead skin cells.


2) Use lemon
The purifying properties of lemon help to remove all the grime buried deep in your pores,  mix a pinch of salt with to get scrubbing effects. Mix lemon juice with a tablespoon of yogurt, a tablespoon of salt, and a bit of honey. Use this to scrub your blackhead-prone areas for 2-3 minutes and then rinse with warm water.

3) Nutmeg and milk
Simply mix nutmeg with half a cup of butter milk to make a thin paste. Starting with a clean face, rinse it once with water, and then begin to apply your exfoliator in light, circular motions. Concentrate on the blackheads, but remember to use a very light touch. Too rough, and you could damage your skin. Do this for 3-5 minutes. Rinse well. Enjoy your clean face.
4) Cornmeal 
Mix a teaspoon of the cornmeal with your favorite face wash and scrub your face gently in a circular pattern. Be caul not to scrub too roughly, as this could inadvertently damage your skin. Rinse the soap and cornmeal off with warm water.

                        Coughing is one of the most common health problems. Most of us know how disrupting, irritating and painful a cough can be. The common cough can be harmless, but a persistent cough can signify a more serious illness. Learn more about the reasons why we cough and how to treat it with a few simple home remedies.

                        When there is a blockage or irritant in your throat or upper air passages, your brain thinks a foreign element is present and tells your body to cough and remove that element. The body uses all power to expel those harmful elements. A cough lex can expel a foreign substance at velocities as high as 100 miles per hour. 

                        It is an extremely useful process and one of the many ways in which the body strives to keep harmful substances out, ensuring you stay healthy. This cough is common cough and is caused by factors such as cold air, irritating fumes, breathing dust, or drawing food into the airways.

                        However, coughing can also be due to a viral infection, common cold, flu, and smoking or health problems such as asthma, tuberculosis and pneumonia. This is called persistent cough and requires medical attention as it may be a symptom of serious illness like cancer etc.

                        How to get rid of common cough naturally.

                        Sputum buildup can lead to a feeling of congestion in the chest and the entire respiratory tract; expelling this sputum theore provides relief from cough and congestion.
                         Some of the symptoms of a cough are itchy throat, chest pain, and congestion. Instead of using over-the-counter cough syrups, you can try some natural cough treatments using readily available ingredients in your kitchen.

                        1) Garlic is  an expectorant, full of antibiotic and antiviral properties, plus garlic helps you cough up stubborn bacteria or mucus that are languishing in your lungs. Experts advise that to reap garlics full cold- and flu-fighting benefits, you have to eat it raw. If you dont like to eat it that way find some ways to mix with your food. or Boil two to three cloves of garlic in a cup of water and add a teaspoon of oregano. Allow it to cool to room temperature, add some honey and drink it. This will help your breathing and alleviate other cough symptoms.

                        2) Honey has long been used in traditional Chinese medicine for coughs because its a natural expectorant, promoting the flow of mucus. This is the simple recipe: Mix 1 tablespoon honey into 1 cup hot water and enjoy. Now how sweet is that? Squeeze some lemon juice in if you want a little tartness.

                        3) Heat a glass of milk, mix in half a teaspoon of turmeric powder and drink warm to find relief from cough. 

                        4) To one cup of hot water, add in half a teaspoon of turmeric powder and half a teaspoon table salt. Use this liquid as a throat gargle and you are sure to experience relief from cough.

                        5) Prepare turmeric tea by adding one tablespoon of turmeric powder into 4 cups of boiling water. Keep for a few minutes, strain and mix in some lemon and honey to the liquid and drink.

                        6) For a dry cough, turmeric powder mixed with a teaspoon of honey taken three to four times a day is effective.

                        7) Take a dried turmeric root, burn it and inhale the smoke. Another way out is to place a few pieces of red hot charcoal in a small mud pot that has been placedon a cup of water. Add a few dried leaves over the charcoal and a spoonful of turmeric powder over the leaves. Gently blow to ignite the turmeric powder and inhale the smoke emitted.

                        8) A glass of hot milk with some honey added just before going to bed is effective at relieving night time cough.

                        9) Mix in honey and lemon juice to a glass of warm water and drink this three times per day for relief from cough.

                        10) Take about 3 tablespoons of flaxseeds in 1 cup of water and boil. When the water starts thickening, strain the liquid and mix in 3 tablespoons each of lemon juice and honey. Swallow about 1 tablespoon of this mixture when you experience a severe coughing 

                        11) Take 10 black pepper corns ( not grounded or powdered pepper please) and boil them in a glass of water. Boil the water till it reduces to half a glass. Filter the pepper corns and allow the mixture to get luke warm. Mix a spoon of honey and take small sips. It will fix the cough for few hours and then repeat. After 2 days of regular use the cough will be gone completely.

                        12) For a dry cough, one of the easiest ways of finding relief is to cut a piece of fresh ginger, sprinkle some salt on it and chew on it for a few minutes. However, not everyone likes the strongly aromatic taste of ginger and in such cases, a tea made with ginger is equally good. Chop ginger into fine pieces, add into a vessel containing one cup of water. Keep boiling the liquid till the volume reduces to half the original quantity. Strain the liquid, add one teaspoon of honey and drink when warm to give a soothing effect against cough.

                        13) Gurgling in salt water always proves to be beneficial in offering relief to an infected or an itchy throat aggravated by coughing. But for an effective change one may resort to gurgling in spinach juice after extracting and warming the same with a dash of salt.

                        14 ) Herbal tea enriched with basil and aniseed may turn out to be soothingly effective as a homemade remedy for sore throat.

                        16) Nutritionally resourceful almonds also play a proactive role in the healing of cough. Paste made out of soaked almonds can be had with a touch of butter for relief from cough.

                        AHRQ Says the Patient Centered Medical Home PCMH Does Not Lower Health Care Costs Heres 5 Reasons To Not Believe Them

                        While you read the following, keep in mind:

                        1. Using traditional research to detect a "statistically significant" decrease in insurance claims expense is notoriously difficult.

                        2. Theres scientific statistical proof and then there is a reasonable business assurance.  The latter may be enough for business-minded health insurers.

                        3. The conclusion is based on published research.  Unpublished "in house" data have convinced many insurers to include the medical home in their covered benefit.  They know something AHRQ doesnt.

                        4. While medical homes alone may be insufficient to save money, that doesnt mean that medical homes that are part of a package of interventions (value-based purchasing, P4P, vendor-based population health management or an ACO) are unnecessary.

                        5. While there is no evidence of savings, there is also no consistent evidence of increased cost. That means the quality associated with the medical home represents high value.

                        That being said, the U.S. Agency for Healthcare Research and Quality (AHRQ) Evidence Report Technology Assessment on the Patient Centered Medical Home says:

                        "Based on a combination of good- and fair-quality studies, there is a low strength of evidence that PCMH implementation may lead to lower utilization (inpatient and emergency department) for some subgroups of patients, but this effect was not uniform. Moreover, total costs were not lowered in the reviewed studies.

                        However, three observational studies specifically designed to test PCMH do report lower inpatient and emergency department utilization among patients in the PCMH program. However, total costs were not statistically different for PCMH and non-PCMH patients in the three studies. None of the clinical trials of functional PCMH interventions had statistically significant differences between intervention and control arms for inpatient or emergency department utilization.

                        Care Management FTE Check Medical Home Training Check But Actually Use It In Patient Care Thats Another Story

                        Months ago, the caffeine-addled Disease Management Care Blog reviewed with the spouse how to pre-set the kitchen coffee brewer for a pot of fresh coffee for the next morning. After listening politely to its earnest instructions, the DMCB spouse rarely set up the brewer.

                        Which is why the DMCB wasnt surprised by an Iowa Chronic Care Consortium (ICCC) study of how its newly minted medical home coach trainees were being deployed in the real world. The ICCC is a not-for profit organization that was founded in 2002 and offers a training program that prepares health professionals to be members of a medical home team.
                        .
                        The Consortium surveyed 318 graduates from their program. Of that number, there were 164 responses, yielding a 54% response rate. 83% of the respondents were nurses, while the remainder were medical or office assistants, diabetes educators, dietitians, social workers, physicians, pharmacists or administrators.
                         
                        The good news is that care-management caffeine was available. The majority of respondents reported that they were using their skills to coach patients for self management, care coordination, planning visits and supporting registry use.  The majority also found their work professionally rewarding.

                        The bad news is that these professionals were not being supported for maximum effect:

                        55% were still "building support for the position."

                        48% reported that "office work" was given a "higher priority," often due to physician and administrator resistance.

                        73% reported that their coaching duties were part time and was in addition to their more traditional roles.

                        Only 8% were involved in office "change management."

                        Only 11% enjoyed a pay differential that rewarded their coaching skills.

                        The Consortiums paper concludes - paralleling the DMCBs coffee-making travails - that a good idea accompanied by well-meaning training is not enough to overcome established clinic routines, business roles and local culture.  The authors recommend that provider payments "change," coaching functions be "operationalized," training programs be "advanced," outcomes be "demonstrated" and that roles be "clarified."

                        While the DMCB ponders coffee change, operations, advances, demonstrations and clarifications, the DMCB has a far more more fundamental concern for the medical home:

                        With friends like this, the fledgling medical home movement doesnt need any enemies.

                        If the clinics that took the time and the money to train these individuals are unable to leverage their skills, disappointing outcomes could end up snatching usual care from the jaws of primary care clinic transformation.

                        The DMCB has three recommendations:

                        History Repeats: Years ago, established "disease management" companies had to go on record and oppose start-ups that were offering "faux" telephonic patient counseling programs. The medical home and population health management community, including the PCPCC, likewise cant afford to have clinics going to market with diluted medical home programs that deliver empty process instead of hard outcomes.  All health care is local, and much of the buy-in for the medical home will ultimately be won or lost at that level.  Speak up!
                         
                        Buyer Beware: Payers and insurers need to be wise to the possibility that having a trained health coach on staff with credentials is not the same as having a health coach on line with the patients. On site credentialing may be in necessary that includes review of policies, job descriptions, pay scales and staff interviews.

                        Build or buy: Primary care clinics need to know that when they buy patient counseling services from a population health management vendor, you get full time on a plug and play basis. That may be a better option for a clinic that is not prepared to both train and fully use an on-site health coach.

                        As Ive said in the past, eating out/ordering in is a colossal waste of money, and beyond the occasional breakdown or special night out I tend not to reach for the Chinese take out menu or make reservations at our local restaurant. My other rationale for not eating out is that I simply will not get the same quality food at 95 percent of the restaurants in our immediate area (including New York City). Im not a trained Chef, but outside some extraordinary restaurants in New York, Ill take a simple meal prepared at home, with high quality ingredients, over a mediocre restaurant experience. Eating well at home is not rocket science, and moreover, you dont always need to purchase exotic ingredients to make tasty dishes. (The ingredients need to be high quality, but they shouldnt break the bank).

                        Here are 4 ingredients that can help you eat cheap and well for as long as our recession lasts:

                        1. Eggs
                        Eggs are a terrific source of protein and can be prepared in a variety of ways. My two "go-to" recipes for eggs include the basic onion and potato frittata and the cheese omelet. I often have eggs for dinner, but aim to keep my egg consumption to 1-2X per week. Also, Ive gone back and forth on using organic/free range vs your typical Omega 3 fortified, Eggland, eggs and to be perfectly blunt there isnt much difference taste-wise. (Eggs taste best fresh so only buy what youre going to consume.)

                        2. Tuna in Olive Oil
                        I always keep between 3-4 cans of Italian tuna in olive oil stocked in my kitchen. My ideal tuna sandwich consists of one can of tuna (do not discard the oil!) with salt and pepper on toasted whole wheat bread. You can also add some fresh parsley and a few slices of tomatoes if theyre in season. (Try it without the mayonnaise, I swear youll like it.)

                        3. Pasta and Rice
                        My pasta and rice arsenal includes: thin linguine, angel hair or capellini, linguine, rigatoni, penne rigate, pastine, soba noodles, arborio rice for risotto, Carolina rice, brown rice, jasmine rice, and whole wheat couscous. Having the aforementioned pasta and rice on hand at all times gives you limitless possibilities, including: linguine with olive oil, parsley, and garlic, baked rigatoni with tomato sauce, mozzarella, and Parmigiano-Reggiano, soba noodles stir fried with green peppers, chicken breast, and onion, brown rice with ginger, cilantro, and cracked black pepper, couscous with feta, red onion, cucumber, and olive oil, etc.

                        4. Whole Chicken
                        Buying a whole, free range, chicken is the equivalent to getting the deal of a lifetime. A whole chicken gets you two types of meat (dark and white) as well various parts which translate into various types of dishes. I purchase a whole, free range, chicken every other week and butcher the chicken according to my perences. If Im interested in making a whole roasted chicken, Ill simply leave the bird as is and make a rub mixture with lemon zest, salt and pepper, rosemary, and olive oil. If I have a little more time on my hands Ill butcher the bird into nine pieces and freeze the parts for various dishes throughout the week, including: braised chicken thighs with fresh mint, breaded and baked drumsticks and wings, thinly pounded chicken breast with shitake mushrooms and sherry wine, etc.

                        10 Ways to Manage Low Back Pain at Home

                        Perhaps you bent the wrong way while lifting something heavy. Or youre dealing with a degenerative condition like arthritis. Whatever the cause, once you have low back pain, it can be hard to shake. About one in four Americans say theyve had a recent bout of low back pain. And almost everyone can expect to experience back pain at some point in their lives.
                        Sometimes it’s clearly serious: You were injured, or you feel numbness, weakness, or tingling in the legs. Call the doctor, of course. But for routine and mild low back pain, here are a few simple tips to try at home.

                        Chill it. Ice is best in the first 24 to 48 hours after an injury because it reduces inflammation, says E. Anne Reicherter, PT, DPT, PhD, associate professor of Physical Therapy at the University of Maryland School of Medicine. "Even though the warmth feels good because it helps cover up the pain and it does help relax the muscles, the heat actually inflames the inflammatory processes," she says. After 48 hours, you can switch to heat if you per. Whether you use heat or ice -- take it off after about 20 minutes to give your skin a rest.
                        Keep moving. "Our spines are like the rest of our body -- theyre meant to move," says Reicherter. Keep doing your daily activities. Make the beds, go to work, walk the dog. Once youre feeling better, regular aerobic exercises like swimming, bicycling, and walking can keep you -- and your back -- more mobile. Just dont overdo it. Theres no need to run a marathon when your back is sore.
                        Stay strong. Once your low back pain has receded, you can help avert future episodes of back pain by working the muscles that support your lower back, including the back extensor muscles. "They help you maintain the proper posture and alignment of your spine," Reicherter says. Having strong hip, pelvic, and abdominal muscles also gives you more back support. Avoid abdominal crunches, because they can actually put more strain on your back.
                        Stretch. Dont sit slumped in your desk chair all day. Get up every 20 minutes or so and stretch the other way. "Because most of us spend a lot of time bending forward in our jobs, its important to stand up and stretch backward throughout the day," Reicherter says. Dont forget to also stretch your legs. Some people find relief from their back pain by doing a regular stretching routine, like yoga.
                        Think ergonomically. Design your workspace so you dont have to hunch forward to see your computer monitor or reach way out for your mouse. Use a desk chair that supports your lower back and allows you to keep your feet planted firmly on the floor.
                        Watch your posture. Slumping makes it harder for your back to support your weight. Be especially caul of your posture when lifting heavy objects. Never bend over from the waist. Instead, bend and straighten from the knees.
                        Wear low heels. Exchange your four-inch pumps for flats or low heels (less than 1 inch). High heels create a more unstable posture, and increase pressure on your lower spine. According to research, nearly 60% of women who consistently wear high-heeled shoes complain of low back pain.
                        Kick the habit. Smoking can increase your risk for osteoporosis of the spine and other bone problems. Osteoporosis can lead to compression fractures of the spine. One study found that smokers are about a third more likely to have low back pain compared with nonsmokers.
                        Watch your weight. Use diet and exercise to keep your weight within a healthy range for your height. Being overweight puts excess stress on your spine.
                        Try an over-the-counter pain reliever. Anti-inflammatory drugs such as ibuprofen (Advil, Motrin), naproxen (Aleve, Naproxyn), and aspirin can help reduce back pain and swelling. Acetaminophen (Tylenol) is another over-the-counter option for pain management. Be sure to check with your doctor or pharmacist about any interactions over-the-counter pain relievers may have with other medications you are taking. People with a history of certain medical conditions (such as ulcers, kidney disease, and liver disease) should avoid some medicines

                        Chocolate Marble Asian Bread Roux Method Home Bakers Challenge 4

                        Tangzhong or roux method give a soft bread,the texture of the bread coz of the water roux aka tangzhong makes cotton like bread and stays pect for more than 2 days in room temperature. But am sure that this bread gets vanished very quickly coz of the presence of chocolate in it.To make the “tangzhong” or “water roux,” a mixture of water and bread flour is heated and stirred just until it forms a paste.  Then, this paste is removed from the heat and allowed to cool, and added to the dough ingredients.  When the water and flour first combine, the flour absorbs the liquid.  Once heat is applied, the flour particles swell many times their original size and burst, releasing starch into the liquid.  At that point, the mixture will turn like gel.This gel like flour paste can then be used as a dough enhancer to improve the moisture and texture in the final bread loaf.Breads prepared by this roux method doesn’t get stale and hard like other breads within a day of baking, it will stays soft and fluffy for several days.

                        Water roux:
                        1/3 cup bread flour
                        1 cup water

                        For dough:
                        2+1/2 cups bread flour
                        2 tbsp sugar
                        1/2tsp salt
                        3tbsp Milk powder
                        100 ml Water + 2 tablespoons water
                        1+1/2tbsp Unsweetened cocoa powder
                        1+1/2tsp Instant yeast
                        2tbsp  Butter

                        Water roux or tangzhong:
                        In a small pan, mix all the ingredients of water roux, place in a low heat and stir constantly, cook until ripples form.

                        Set aside to cool by covering with a plastic film.

                        Dough:
                        In a bowl, add the water roux, and all the other ingredients, except for the butter, water and cocoa powder.
                        knead it until it forms soft dough.

                        Add the butter and  knead until the butter incorporates into the dough.

                        Divide the dough in approximately 2/3 and 1/3 portions. Remove the small portion and set aside by covering with a cling film.

                        Knead the bigger portion until the dough is smooth and elastic. Set aside in a bowl by covering the dough with a cling film.

                        Take back the small portion and knead it again and add cocoa powder and water as needed and knead until the dough is smooth and elastic.

                        Cover the chocolate dough and set aside in a bowl by covering with a cling film.Let both dough proof until the dough doubles to its original size.

                        Knock back both dough and let it rest for 5 minutes on the counter, cover both dough.


                        Flatten the chocolate dough into a rectangle shape of approximately 6 x 9 inches and the white dough to 12 x 6 inches.

                        Layer the chocolate rectangle in between the white rectangle. Flatten the rectangle and cut in 2 equal portions. Layer one piece on top of the other and flatten again.

                        Repeat this procedure 3-4times.

                        Then again cut into 3 equal strips. Gently roll the strips and place 3 together and make a braid.

                        Place the braided bread in the loaf tin. Cover the tin with plastic film and let them rise in a warm place until the dough has almost tripled its original size.

                        Meanwhile, preheat oven at 350 F.Bake the tin for approximately 20 minutes.

                        Remove from the oven and brush with melted butter.

                        Transfer the bread to a wire rack to cool.

                        Slice and enjoy with your favourite spread.

                        Read this New England Journal Perspective by Drs. Ouslander and Berenson on Reducing Unnecessary Hospitalizations of Nursing Home Residents, and its easy to conclude that the enlightened application of better documentation, quality assurance, payment orm, administrative policies, decision support, guidelines, appropriate downjobbing and delegation of responsibility will keep many sick and feverish nursing home residents away from the unnecessary expense of emergency rooms and hospitals.

                        Were that it was so simple.  Such is the unreality of many Journal authors inhabiting the oxygen-deprived academisphere.

                        When residents of nursing homes get sick, they require a significant amount of nursing attention.  Since busy, salaried (and sometimes understaffed) nursing home personnel arent compensated for the additional work of documenting, monitoring and worrying about that sick patient down the hall, they have every incentive to transfer sick patients elsewhere.  No amount of macroeconomic policy making bells and whistles is likely to change that.

                        When physicians and other non-physician providers are alerted by the nursing home personnel that a patient is sick, they have been taught that the best approach is to evaluate the patient in person. That takes time and effort.  While they may be compensated for the additional work of a bedside evaluation, its not enough to make up for the additional work of documenting, monitoring and worrying about that sick patient down the hall.  So, they also have every incentive to agree with the nurse and transfer the patient to a setting where a bedside evaluation is available.  At 2 AM, the answer is to move that bedside to the local emergency room.

                        Would the bells and whistles that Ouslander and Berensen be enough to keep the nursing homes and the doctors from sending most sick patients to the emergency room?  Based on the simple realities described above, the answer is no.
